01

Systems Thinking

Hormones don’t work in isolation. Learn the HPATG framework that makes every case start making sense.

02

Clinical Precision

Not just what to test: when, why, and exactly how to interpret it. Testing strategy that’s actually strategic.

03

Real-World Application

Every module builds toward practice. You leave with frameworks, tools, and protocols you can use immediately.

Sound familiar?

You have the education. You still have the gaps.

I was that person who did all the certifications. Multiple degrees too, trying to fill the gaps with more knowledge. I love to learn, so it felt productive. It was a trap. None of it taught me what to do when there was a real human being in front of me who didn’t fit the pretty algorithm. Someone who wasn’t spending $2,000 on labs and wasn’t going to run the protocol exactly the way I’d been taught it.

  • Clients with clear hormone symptoms and labs that come back “normal”
  • Protocols that work for some clients and not others, with no clear reason why
  • Thyroid cases where TSH is fine but the patient is not
  • Women in perimenopause being told their symptoms are just anxiety
  • The feeling that you’re missing a framework that ties it all together
  • Certifications that gave you information but not clinical thinking

You’re not alone

Almost no one is teaching practitioners how these systems work together.

And without that, even the best education leaves you piecing things together and hoping it adds up.

What’s actually been missing:

  • The HPATG framework as one integrated system, not isolated axes with isolated solutions
  • A testing strategy, not just a list of test options
  • Clinical protocols mapped to root cause, not just symptoms
  • The confidence to handle complex, multi-system cases

The difference

This is what changes everything.

Most training teaches you to test estrogen and progesterone and see what needs fixing. This teaches you to zoom out. For this person, where is it actually coming from? Chronic stress? Perimenopause? Something sitting underneath both? That shift is the whole certification.

When you understand how hormones work as one connected system, you stop chasing symptoms and start finding patterns. You stop guessing and start knowing.

Dr. Laura DeCesaris
With this certification

What becomes possible.

Walk into any hormone case with a clear framework, not a checklist
Read a DUTCH panel the way it was designed to be read
Finally understand why TSH is the wrong starting point for thyroid
Know exactly when HPA dysfunction is upstream of everything else
Build PCOS, endo, and fertility protocols based on the actual root cause
Guide perimenopause patients who’ve been dismissed for years
Use BHRT and peptides with clinical confidence, not guesswork
Build a practice known for getting the results others can’t
Everything you’ll receive

A comprehensive clinical training designed to permanently change how you practice.

This isn’t a course you consume and forget. It’s a comprehensive clinical training with the education, support, and community to make sure it actually sticks.

🏫

15 Core Modules

Each module runs 6–8 parts: the physiology, the testing, the protocols, then real case studies and the coaching skills to get a client to actually follow through. Including 2026 clinical updates, like where GLP-1s fit.

4 Bonus Modules

Gut-hormone connection, nitric oxide, male hormone blueprint, and women’s cardiovascular health.

📄

Clinical Tools & Templates

Intake forms, case-taking frameworks, testing decision trees, plus the sequencing logic. Knowing what to do matters less than knowing what to do first, and that’s the part nobody teaches.

👥

Community & Support

Ongoing access to a community of practitioners working at the same level, with monthly live Q&A and mentorship support.

What you’ll learn:

How it’s structured.

Every module builds on the one before it. By the time you reach clinical protocols and advanced therapeutics, you’re not just applying steps. You’re thinking in systems.

01
The Foundation
Why the HPATG system changes your entire clinical approach, and how to test it without guessing
Most practitioners were never taught to see hormones as a system. This is where that changes.

Before you interpret a single lab or build a single protocol, you need the right map. The foundational modules establish the HPATG framework and the testing strategy that makes every clinical decision that follows actually make sense.

Module 1: A Systems-Biology Approach to HormonesThe HPATG framework and the 3 Laws of Hormones: Hierarchy, Goldilocks, and Pleiotropy. This is the clinical lens that makes symptoms predictable instead of random, and the foundation every other module in this certification is built on.
Module 1.5: Clinical FoundationsIntake structure, case-taking frameworks, and the clinical reasoning habits that bridge textbook physiology to real practice. The piece most training programs skip entirely.
Module 2: Functional Hormone TestingWhen DUTCH tells you more than serum. When saliva is actually appropriate. How to build a testing hypothesis before you order a single lab, and exactly how to interpret what comes back.
02
The Stress Axis
Four distinct cortisol patterns, why each needs a completely different fix, and why nothing else heals until this does
Adrenal dysfunction is upstream of almost everything. Until you address it, nothing downstream fully heals.

Adrenal dysfunction is upstream of thyroid suppression, progesterone depletion, and insulin dysregulation. Until you address the HPA axis, everything downstream is working against you.

Module 3: The HPA Axis and Stress Response (The Why)The physiology of cortisol as master regulator of the entire hormonal hierarchy. The four dysfunction patterns (High, Low/Exhaustion, Flattened, Reversed), and why misidentifying the pattern means applying a protocol that makes things worse, not better.
Module 4: HPA Axis: Testing + Healing ProtocolsHow to read a 4-point diurnal cortisol curve and what it tells you that a single morning draw never can. Targeted supplement, lifestyle, and clinical protocols mapped specifically to each dysfunction pattern, not a generic adrenal support protocol.
03
The Thyroid
Beyond TSH: the conversion issues, Reverse T3 patterns, and autoimmunity that most standard panels miss entirely
Most thyroid patients have been told they’re normal. Here’s how to find what standard testing misses.

Conversion problems, silent autoimmunity, functional deficits sitting inside “normal” reference ranges. These two modules give you the tools to find and address what conventional testing is designed to miss.

Module 5: The HPT Axis and Thyroid Health (The Why)Why TSH only tells you what the pituitary is doing. The physiology of T4-to-T3 conversion, why Reverse T3 accumulates and what it means clinically, and how Hashimoto’s can be actively destroying thyroid tissue for years before TSH moves out of range.
Module 6: HPT Axis: Testing + Healing ProtocolsHow to run and interpret a complete functional thyroid panel (TSH, Free T4, Free T3, Reverse T3, TPO and TgAb antibodies), using functional reference ranges that reflect optimal health, not just the absence of disease. Protocols that address conversion and autoimmunity, not just hormone replacement.
04
Female Hormones
Cycling hormones, PCOS, endometriosis, fertility, perimenopause, and the full menopausal transition, in real clinical depth
The most comprehensive female hormone training in any certification program, from first cycle to menopausal transition.

Five modules covering the full arc of female hormonal health. Not a survey. A deep clinical dive into every phase, with the testing strategy and protocol detail that most certifications don’t come close to.

Module 7: Menstrual Cycle & Sex HormonesFollicular and luteal phase physiology, estrogen metabolism pathways, and progesterone’s role beyond reproduction. Why testing timing relative to cycle phase completely changes how you interpret results, and what most practitioners get wrong because of it.
Module 8: Reproductive Years: PCOS, Endometriosis & FertilityFunctional assessment and protocol frameworks for the three presentations most commonly seen in practice. PCOS subtypes and why they require different approaches. Endometriosis through the lens of estrogen metabolism. The hormonal picture underlying fertility challenges.
Module 9: Perimenopause: The WhyHow the HPG axis begins its transition, and how that simultaneously destabilizes the HPA and HPT axes. Why perimenopause produces such a wide and confusing symptom picture, and why it gets misread as anxiety, depression, or thyroid disease when it’s none of those things.
Module 10: Perimenopause: Testing + OptimizationWhat testing actually tells you in perimenopause, and what it doesn’t. Progesterone support, estrogen balance strategies, and the clinical and lifestyle interventions that make a real difference during the transition.
Module 11: The Menopausal TransitionWhat the WHI study got wrong, what current evidence actually supports, and how to build individualized hormone support decisions for each woman, based on her history, her risk profile, and what she actually needs.
05
Advanced Practice
Insulin resistance as the metabolic foundation, BHRT without shortcuts, peptide therapy, and a complete clinical toolkit
This is where everything comes together: the advanced therapeutics, the metabolic foundation, and the tools to use it all starting Monday.

The capstone of the core curriculum. Four modules that cover the metabolic underpinning of nearly every hormone case, the advanced therapeutics most certifications won’t go near, and the clinical systems that make all of it immediately usable in practice.

Module 12: The Metabolic Core: Insulin ResistanceHOMA-IR, fasting insulin, HbA1c, and lipid pattern assessment as metabolic markers. Why insulin resistance is upstream of estrogen dominance, testosterone dysregulation, and thyroid conversion problems, and why hormone protocols fail when this isn’t addressed first.
Module 13: BHRT & Advanced TherapeuticsBioidentical hormone types, delivery methods, and dosing frameworks. When BHRT is appropriate, when it’s not, and how to use it correctly, meaning after the foundational work that determines whether it will actually hold.
Module 14: Peptides & LongevityClinical applications of peptide therapy within a hormonal health and longevity framework: the science, the current evidence, and how to think about integrating them into a systems-based practice.
Module 15: Practitioner’s Clinical ToolkitIntake forms, case-taking protocols, clinical decision frameworks, and the implementation systems that make everything actionable. You leave with a complete practice infrastructure, not just knowledge of what to do.

My Background

I don’t teach a method. I teach you how to think.

I’ve been in the functional and integrative health space for over a decade, and I still see clients. I’m in the trenches on purpose. You lose something when you stop working with patients, and you forget what it’s actually like to sit across from a woman who’s already been failed twice before she got to you. Everything in this certification came out of those cases, not out of a curriculum.

That’s why I built the HP Mentorship. And that’s why this certification exists within it, because education without a mentor to ask when cases get hard, and a community to practice alongside, isn’t education you’ll actually use.

My Background:
  • Cornell University undergraduate
  • Doctor of Chiropractic  |  Clinical Nutrition
  • Institute for Functional Medicine (IFM) Certified
  • American Academy of Anti-Aging Medicine (A4M) Fellowship
  • Featured in Forbes, NBC News, mindbodygreen, Parade, Well+Good

Who this is for

Is this for you?

This is for you if…

  • You’re a clinician, coach, nutritionist, or functional health practitioner working with women at any stage of practice
  • You’re tired of cobbling together your education from disconnected sources and want one coherent clinical framework that actually connects the dots
  • You’re seeing complex cases (PCOS, thyroid dysfunction, hormonal imbalance, burnout) and want to meet them with precision instead of uncertainty
  • You know your clients aren’t paying for the protocol; they’re paying for you to guide them through it. You’re the magic. This sharpens the clinician doing the guiding.
  • You’re ready to invest in clinical mastery, not just another credential to add to your bio
  • You’re new to functional medicine and want to learn the right framework from the beginning

This probably isn’t for you if…

  • You’re looking for a cookbook, a set of protocols to apply without understanding the physiology behind them
  • You want passive, consume-and-forget education. This program requires active thinking, application, and real engagement
  • You’re not yet ready to shift from symptom-chasing to systems thinking. This is the advanced lane, and it demands commitment
  • You want a quick credential with minimal depth
Ready to enroll?

Your investment in clinical mastery.

When you think about what it costs to piece this together on your own (the courses, the conferences, the trial and error in practice), this is not a large number.

  • 15 comprehensive modules, with video lectures, case studies, frameworks, and clinical reference materials
  • 4 bonus modules: Men’s hormones, gut-hormone connection, nitric oxide, cardiometabolic health
  • Monthly live Q&A call. Bring your questions from the material and get them answered
  • Slack-based support community. Ask questions and connect with other practitioners in the program
  • Women’s Health & High Performance Specialist designation upon completion

Questions & answers

A few things people usually want to know.

Do I need to be a licensed healthcare provider to enroll?

Not at all. The certification is open to clinicians, coaches, nutritionists, and functional health practitioners at every stage, from those just entering the integrative space to experienced practitioners looking to deepen their foundation. If you’re committed to learning and applying this material at a clinical level, you’re a fit.

Is this part of the HP Mentorship, or is it standalone?

This is a standalone certification, though members of HPM automatically get access to the certification as part of their enrollment.

What’s the time commitment?

The modules are self-paced. Most practitioners move through the core curriculum in 8–12 weeks depending on schedule. This is dense, clinical material. Budget time to absorb and apply it, not just watch it. The framework is only useful if it becomes the way you actually think.

What do I receive when I complete the certification?

You’ll earn the Women’s Health & High Performance Specialist designation, a credential you can use in your bio, on your website, and in your marketing to signal your expertise to ideal clients. Plus a certificate of completion. And most importantly: a clinical framework you’ll use for the rest of your career.

Is there mentorship and community included?

Great question! Every month there is a live Q&A call where you can bring your questions directly from the material. You also get access to a Slack community of practitioners going through the same program, so you’re never working through it alone. And if you’re looking for deeper ongoing mentorship, monthly case studies, office hours, guest coaching, and a full practitioner community, that’s exactly what the High Performance Mentorship is built for. Many certification students choose to join HPM alongside or after completing the certification. You can learn more about the HPM here.

What makes this different from other hormone health certifications?

Most hormone training teaches you what to do when everything goes according to plan. It doesn’t teach you what to do when it doesn’t: when your client plateaus, or gets worse on a protocol that should have worked, or won’t pay for the panel you wanted.

So this is built differently. You work named case patterns you’ll actually recognize: the tired-but-wired executive, HPA compensation in the high achiever, sluggish clearance. Every module includes the coaching side too, because knowing the protocol doesn’t help if you can’t get someone to follow it. And it teaches the HPATG framework: how the stress, thyroid, and sex hormone axes actually talk to each other, so you can zoom out and see where a picture is really coming from.

Not what to do. How to think.

What if I’m new to functional medicine?

This is actually a wonderful place to start, because you’ll learn the right framework from the beginning instead of having to unlearn fragmented approaches later. The curriculum starts with foundations and builds systematically. If you’re ready to commit to learning at a clinical level, you’re ready for this program.

There is a way to practice where the complexity doesn’t stop you.

Where a challenging case walks in and you feel ready, not anxious.

Where your labs tell a story you know how to read. Where your clients come back not because they’re still searching for answers, but because they found them with you.

You know more than you think you do. You do. What’s missing isn’t knowledge. It’s the map that shows how the pieces connect, and the reps to practice on. Most practitioners never get handed either one. They get the pieces, then spend years trying to assemble the puzzle alone.

This is the map.

And the community, the mentorship, and the clinical tools that help you use it.
